 GM Business Choice is an incentive designed specifically for business owners who use vehicles in the day-to-day operation of their business and not solely for transportation purposes. This incentive is available with eligible Chevrolet vehicles and offers valuable upfit equipment or card option choices shown below.

Business owners who can provide one of the following eligible business documents can take advantage of the GM Business Choice Program. All eligible business documentation must link the owner's name to the business.

• State Business License or State/Federal Business Tax ID Certificate
• GM Fleet Account Number (FAN)
• Prior-year Schedule C (Form 1040) or Schedule F (Form 1040 - Farmers ONLY)
• Sales Tax License
• DBA (Doing Business As)
• State Occupational License for Trade (For example, plumbers, electrician, contractors, etc.)
• State or Federal Authorized Business website (must have .gov website address)
• Municipal Business License
• Commercial General Liability Insurance Policy/Public Liability Insurance Policy and/or Workmen's Compensation Insurance Policy

1. Option D: The 2011 GM Business Choice Program is valid from October 3, 2011, through September 30, 2012. Every time you lease or purchase a new, eligible Chevy or GMC vehicle and choose Option D within the GM Business Choice program at participating GM dealerships, Chase will post a $500 statement credit to your account. You must purchase or lease your vehicle during the program period and be prepared to provide proof of your business. Vehicle must be used in the day-to-day operation of the business and not solely for transportation purposes. This program may not be compatible with other offers or incentive programs. 2. If you are already a GM Business Card holder, you do not need to re-apply. Simply call the number on the back of your card and provide your claim number to the Chase Advisor in order to receive your statement credit. Earnings may not be combined with other certain targeted fleet, commercial or recreational programs. Please see your GM Business Card Program Rules that you received at the time when your account was opened. Applicants not meeting credit requirements will receive a $500 Lowes Gift Card or $500 Customer Rewards card chosen at the time of claim filing process. 3. Participation in the program allows you to enjoy the benefits of the GM Business Card World MasterCard®, including the ability to accumulate "Earnings". You will accumulate Earnings at a rate of 5% for each $1 of net purchases made on dealership parts and service at authorized GM Vehicle Dealers. You will accumulate Earnings at a rate of 3% for each $1 of net purchases made at merchants that classify their merchant locations for MasterCard in any of the following categories: dining establishments, office supply stores and wholesale distributors, gas at the pump. You will accumulate Earnings at a rate of 1% for each $1 of all other net purchases. We do not determine whether merchants correctly identify and bill transactions as being made at locations in each particular category. However, we do reserve the right to determine which purchases qualify for the 3% Earnings. Purchases not eligible to receive 3% Earnings include, but are not limited to, purchases made at superstores, warehouse clubs, discount stores, and their affiliates. Online and catalog purchases are not eligible to receive 3% Earnings unless the merchants have correctly identified and billed the transactions. You do not accumulate Earnings on balance transfers, cash advances, cash-like charges such as travelers checks, foreign currency, and money orders, any checks that are used to access your account, overdraft advances, interest, unauthorized or fraudulent charges, or fees of any kind, including fees for products that protect or insure the balances of your account. 5. Option A, B, C or E: You will qualify for and receive your bonus after your first use of the card. First use includes using your account for purchases or balance transfers, or using any checks that access your account; cash advances are not included. After qualifying, please allow 6 to 8 weeks for statement credit to post to your account. To be eligible for this bonus offer, account must be open and not in default at the time of fulfillment.
